Course Change Form

This Course Change Form is intended to assist participants who are requesting a transfer from one registered OLGH CGS course to another after either course has started. Such requests are considered carefully and prayerfully, with respect for the formation journey of the participant, the work of the Formation Leader, and the well-being of both courses involved.

All course change requests are subject to approval by the Formation Leader. Submission of this form constitutes acknowledgment that the Course Change Fee of $100* is non-refundable once approved; that the participant is responsible for all missed coursework and presentations; that the syllabus for the requested course must be submitted; and that only one course transfer is permitted.

*OLGH CGS Members with a current Good Shepherd or True Vine Level membership pay a reduced Course Change Fee of $50 and must be logged in to their member account to submit this request form. Members who do not log in will be charged the full $100 fee and will not receive a refund for the difference.
